Concerns About Patient Care and Safety

Your concerns are important to us and we want to provide you with quality and safe treatment services. We would like for you to discuss your concerns with your treatment team professional. If you or your family have concerns that are not addressed by your clinical staff member we encourage you to contact the LSCI Clinical Director or the Complaint Line.

If your concerns are not resolved internally by our organization, you may notify The Joint Commission about such matters. The Joint Commission deals with organizational quality, safety-of-care issues and the safety of the environment in which care is provided. Such reports should be addressed to:
